Patients without medical insurance will be required to provide a deposit before their

initial office visit or procedure. To discuss account balances and payment arrangements, please contact our Billing Department at (804) 330-4901.

  • Referrals

If you are enrolled in an HMO plan, please contact your insurance carrier to determine

whether a referral is required before scheduling an appointment with a specialist.

If a referral is required, it is the patient's responsibility to coordinate with their primary

care provider and insurance carrier to obtain the necessary authorization before the

    appointment.

Patients who do not obtain a required referral may be asked to sign a waiver

acknowledging financial responsibility for services not covered by their insurance plan.

  • Patient Financial Responsibility

Patients are responsible for any applicable co-payments, deductibles, co-insurance, and

other out-of-pocket expenses required by their insurance plan. Payment is due at the

    time of service.

We accept most major credit cards, cash, personal checks, and contactless payment

options, including Apple Pay and Google Pay.

Questions regarding insurance benefits or coverage should be directed to your

    insurance carrier.

  • Procedure Billing Information

Endoscopic procedures are performed in an outpatient setting, either at one of our

endoscopy centers or at a hospital outpatient facility.

Depending on your procedure and where services are provided, you may receive

    separate bills for:

    • Physician services
    • Anesthesia services
    • Pathology services (if tissue samples are collected)
    • Facility charges

These services may be billed separately and processed independently by your

    insurance carrier.

If you have questions regarding insurance coverage or anticipated out-of-pocket

expenses, please contact your insurance carrier prior to your procedure.

  • Verify Your Coverage Before Your Procedure

Before your procedure, it is important that you understand how your colonoscopy will be

classified. Please request the pre-procedure diagnosis code (reason for the procedure)

from the person scheduling your procedure and contact your insurance carrier to

    confirm:

    • Whether a colonoscopy with this diagnosis is covered under your plan
    • Whether it will be processed as preventive or diagnostic
    • If diagnostic, whether charges will apply to your deductible and, once met, to coinsurance
    • Any estimated out-of-pocket costs under your plan

If your procedure is scheduled at Virginia Endoscopy Group – North, please inform

your insurance carrier that the procedure will be performed in an office-based

    outpatient setting.

If your procedure is scheduled at Richmond Endoscopy – Westchester or West

Creek Endoscopy Center please inform your insurance carrier that the procedure will

be performed in an Ambulatory Surgery Center (ASC) setting.

If your colonoscopy is expected to be classified as diagnostic and applied to your

deductible, please contact our Billing Department at (804) 330-4901 for an estimate of

    your financial responsibility.

Please understand that our physicians cannot change a diagnosis solely for

insurance coverage purposes. The diagnosis must accurately reflect your medical

history and the information provided during your pre-procedure assessment. This

requirement is mandated by federal regulations and insurance contract guidelines.

  • Patient Account Updates

    • You insurance coverage changes.

Colonoscopy Coverage Information

The Affordable Care Act (ACA) requires that recommended preventive services, such as screening colonoscopies, be covered by most insurance plans at no cost to the patient. However, insurance companies apply specific guidelines to determine whether a colonoscopy is classified as preventive, diagnostic, or surveillance/high-risk.

Review the information below to better understand how your colonoscopy may be classified and how that classification may affect your insurance coverage and out-of-pocket costs.

  • Screening Colonoscopy / Preventive

A preventive or screening colonoscopy is typically performed when the patient:

    • Has no current gastrointestinal symptoms
    • Is age 45 or older (or meets other recommended screening guidelines)
    • Has no personal history of gastrointestinal disease, colon polyps, or colorectal cancer
    • Has not had a colonoscopy within the past 10 years

Even if another provider refers you for a screening colonoscopy, the final classification is based on your documented medical history. To qualify as a preventive screening, there must be no prior history or current symptoms.

  • Diagnostic Colonoscopy / Therapeutic

A diagnostic or therapeutic colonoscopy is performed to evaluate or treat:

    • Gastrointestinal symptoms
    • Colon polyps
    • Known or suspected gastrointestinal disease
  • Surveillance Colonoscopy / High-Risk

A surveillance or high-risk colonoscopy is typically performed when the patient:

    • Has no current gastrointestinal symptoms
    • Has a personal history of gastrointestinal disease (such as diverticulitis, Crohn’s disease, or ulcerative colitis), colon polyps, or colorectal cancer

These patients often require more frequent colonoscopies, typically every 2–5 years.

Insurance coverage for surveillance colonoscopies varies; some plans consider them preventive, while others classify them as diagnostic.
